4. “Personal Information” shall mean and refer to any personally identifiable information that has been submitted by you or,we may ask / or collect from you

5. “Third Parties” refer to any application, website, company or individual apart from the User and the creator of the Application.

  1. OVERVIEW

In order to use the services of this Application, You are required to register yourself by verifying the authorised device (verified by OTP (one time password) on MOBILE NUMBER OR EMAIL ID provided by you). This Privacy Policy applies to your information that we collect and receive as submitted by you on and through Telidox; it does not apply to practices of businesses that we do not own or control or people we do not employ.

As a User of Telidox, you may provide information about yourself, your spouse or family, your friends, their health issues, gender of the patient, age of the patient, previous medication taken, previous medical conditions, allergies, drug reaction, known complications, history of hospitalisation/ surgeries performed, past and existing chronic disease etc.

The Information specified above and collected by us may be classified as ‘Personal Information’ or ‘Sensitive Information’ under the Information Technology (Reasonable security practices and procedures and sensitive personal data or information) Rules, 2011. Collection of information which has been designated as ‘sensitive personal data or information’ under the Information Technology (Reasonable security practices and procedures and sensitive personal data or information) Rules requires your express consent. By affirming your assent to this Privacy Policy, you provide your consent to such collection as required under applicable law. Our Services may be unavailable to you in the event such consent through an OTP and/or email is not given.

For HEALTH CARE PROVIDERS (HCPs) /Practitioners using Telidox, we would collect your Name, Education, Specialization, Registration Number registered with The Medical Council, Annual Practising License, Good Conduct Certificate, Declaration Of No Criminal Or Civil Case Pending Against You Of Any Kind in any court of law or any Attachment With Medical Institution/Colleges address of practising hospitals/clinics, Contact information, achievements, Device ID (IMEI), Location, etc. If you use Telidox by accessing it through your Google and Facebook Profiles and other API’s, we collect such data as well.

MAKE SIMILAR POINT FOR PHARMACIST ( INCLUDING RETAIL DISTRIBUTER AND DRUG COMPANIES ALIKE), DIAGNOSTIC CENTRE AND LAB.WE ARE GIVING SOME POINTERS TO BE ADDED, ADD THE REMAINIG NECESSARY ONES

For PHARMACIST/PHARMACY which includes E-PHARMA COMPANIES, MANUFACTURERS, AND ALL OTHER ASSOCIATED with PHARMA Industry using Telidox, we would collect Owner’s/Registered Pharmacist’s Name, Education, Specialization, Authorities Number, Address Of Pharmacy Shop, If you use Telidox by accessing it through your Google and Facebook Profiles and other API’s, we collect such data as well Registration Number registered with The Pharmacy Council, Good Conduct Certificate, Declaration Of No Criminal Or Civil Case Pending Against You Of Any Kind in any court of law, Contact Information, Achievements, Device Id (Imei), Location, Etc. and Also Shop Details Like Drug License Number, Shop Act Registration With Local.

FOR LABS/DIAGNOSTIC CENTRES/HOSPITALS/MEDICAL INSTITUTIONS/NGOs using Telidox, we would collect Owner’s/Registered Doctor’s Name, Education, Specialization, Registration Number registered under the Concerned Medical Council, Good Conduct Certificate, Declaration Of No Criminal Or Civil Case Pending Against You Of Any Kind in any court of law, Contact Information, Achievements, Device Id (IMEI), Location, Etc. and Also Centre Details Like Radiation Control Authorities/ Atomic Energy Board Or Registration Essential With Any Other Allied Government Authority,Facility Registration Number With The Local Bodies, Registration Number, address of the establishment Shop Act License, If you use Telidox by accessing it through your Google and Facebook Profiles and other API’s, we collect such data as well.


 

FOR using Telidox, we would collect OWNER’S/REGISTERED DOCTOR’S Name, Education, Specialization, Registration Number registered under the Pharmacy Council, Good Conduct Certificate, Declaration Of No Criminal Or Civil Case Pending Against You Of Any Kind in any court of law, Contact Information, Achievements, Device Id (Imei), Location, Etc. And Also Hospital Details Like Drug License, SHOP REGISRATION NUMBER (With local authorities such as municipal authorities) , address of PHARMACY SHOP, If you use Telidox by accessing it through your Google and Facebook Profiles and other API’s, we collect such data as well.

  1. INFORMATION COLLECTION AND USE

For a better experience, while using our Service, we may require you to provide us with certain personally identifiable information, including but not limited to your name, address, e-mail address and landline/mobile number, age, username, password and other registration information, financial and credit card information, personal description and photograph.( work on device phone details )

The information that we request will be retained by us and used as described in this privacy policy.

The app does use third party services that may collect information used to identify you.

The information made available by Patients is displayed to Practitioners and vice versa. The information collected from Practitioners is displayed on the Application to enable a User to connect with a Practitioner of their choice.

  1. DISCLOSURE OF YOUR INFORMATION TO THIRD PARTIES:

When you use our Application, we must provide some of your personal information to third parties (likeyour name, address, e-mail address and phone number, personal description and photograph, educational qualification, copies of you degrees – (Why is this document of patient required to be shared) and other professional qualification, language spoken, other data required to be used for/between the patient and the Healthcare Provider (HCP)) to give you better services and for enhancement and visibility of our Application. -

However, we do not sell or rent individual customer names or other Personal Information to third parties except sharing of such information with our alliance partners or vendors who are engaged by us for providing various promotional and other benefits to our customers from time to time.

The sections we refer to Section 43A of the Information Technology Act, 2000;Regulation 4 of the Information Technology (Reasonable Security Practices and Procedures and Sensitive Personal Information) Rules, 2011 (the “SPI Rules”);Regulation 3(1) of the Information Technology (Intermediaries Guidelines) Rules, 2011, as applicable in India.

Due to the existing regulatory environment, we cannot ensure that all of your information shall never be disclosed in ways other than those described in this Privacy Policy. For example, but without limiting and foregoing, we may be forced to disclose Your Personal Information to the government, law enforcement agencies or other Third Parties, in India or overseas. Under certain circumstances, Third Parties may unlawfully intercept or access transmission or private communications, or abuse or misuse Your Personal Information that they may collect from our Application. Therefore, we do not promise, and you should not expect, that your personally identifiable information or private communications would always remain private. We may share your personal information to those service providers and Health Care Providers (HCPs)with which you make arrangements through our website. We provide them with the Personal Information needed to make and complete the appointment / answer the query you pose. We strictly restrict any kind of direct communication SUCH AS e-mail, mobile number OR OTHERS, between the patient and the user and any service provider outside Telidox. If any direct communication is used by any of the parties his registration shall be removed/ cancelled and he/she shall be liable to be punished as per law. No further services will be provided to him /her by the Telidox, WITH IMMEDIATE EFFECT.

1. We may share our data, including Your Personal Information, with our parent and/or subsidiaries that are committed to serving your needs through use of our Application and related services, throughout the world..

2. We cooperate with law enforcement inquiries, as well as other third parties to enforce laws, such as: intellectual property rights, fraud and other rights.
